The content is divided into three levels of proficiency: Beginner, Intermediate, and Advanced, with an additional mixed-level option that provides a comprehensive practice experience. Each case follows a question, diagnosis, and rationale format and includes hyperlinked references and supplementary materials. The authors plan to expand the collection by adding hundreds more cases over time.

The primary author, Ying Chi Yang, and co-author, Rezwan Munshi, are certified internists by the American Board of Internal Medicine and currently serve as cardiology fellows at Trinity Health-MercyOne North Iowa, USA.
